Anyone who needs a DVD or VHS tape duplicated for academic purposes should complete a media request form. The requestor must supply the blank media. Be sure that your request includes:

• The date and time the person will arrive with video data to be copied
• The date copies need to be completed
• A brief description of the video data that is being copied

The lead time required to meet a request depends on the number of needed copies:

• 1 to 3 copies / one week
• 4 to 10 copies / 1 month
• 11+ / only done between semesters
